Be sure to upload as many documents as possible to the website.  You will need the following to register:

Documents needed for Registration:

a.    Proof of Residency:

• i.     Lease, sales contract, settlement statement, warranty or quick claim deed, or residential property tax statement
•  ii.     Utility Bill – Water or Electric
•  iii.     Residency Affidavit if your name is not on the contract. The homeowner will need to bring their lease/sales contract and utility bill. The residency affidavit MUST be signed by the homeowner and the enrolling parent and be notarized at the SCHOOL.                                           

b.   Official document showing proof of birth of student

c.    Photo ID of Parent/Legal Guardian

d.   Updated Immunization Form 3231 (on GA form)

e.    Vision, Hearing, Dental and Nutrition Form 3300

f.     Social Security Card
